FYI, it seems to also happen when you filter by user. It may happen if you filter by anything.

Ah, mystery solved. Without the date filter, it shows the paths with the date filter, no paths just the filename. Does everyone see this or is there some configuration we can change to work around it? We can always use no date filter for the time being, but that is a little slower. We are using 2.0....

I am doing by item history on a parent project and I don't see the full path to the sub project for each file. I attached a screen shot so you could see what I was talking about.
