I'm not exactly sure how they did it, but I believe they added a file to the same folder with the same name. They mentioned doing a share and then changing the name. The scenario when I came in was that there were two files with the same name in the same folder. One had 196 revisions the other had 1...

Problem committing and operation with duplicated file
Item $/Clients/customer.xls caused the transaction to fail: The object could not be found. It may have been deleted, moved or renamed. How do I get this file back to where I can use it? It was duplicated and the duplicate was deleted. After that then this error began popping up with the original.

Fixed by changing the source provider
We had this same error with Fortress and Vault installed. The project had fortress bindings, but VS2005 was set to use vault. We changed to fortress and everything worked fine.
