Vault Client User Randomly denied access to Repository

We have one user who will randomly be denied access to a repository. This is Vault 5.0.3.18802 .
It will occur when moving between repositories. Usually, switching back to another repository and flipping back again clears it.
She is a 'Full Admin' with those rights.
One thing is that we are maxed out on licenses. We have 25 of our 25 in use.
Also, when she and another developer 'log out' the admin web portal still shows them both as logged in. That never changes it seems.

Not serious, but annoying.

What's the exact error that the user gets?

We'd like to see a copy of the Vault Server log after she gets this error. It's called sgvault.log and is in %windir%\temp\sgvault on the server machine.
